A Pediatrician with Knowledge and Heart
Dr. Jennifer Perez, MD, FAAP, DABOM is a native Chicagoan. Born to Cuban immigrant parents, she always dreamed of being a Pediatrician. She graduated from Loyola University with a degree in Biology, went on to Medical School and the University of Illinois at Chicago and Residency at Rush University Medical Center.
She has been a primary care pediatrician in private practice since her Residency graduation in 2009. She has continued her studies; first in Lactation Consultant, then Board Certification in Obesity Medicine, and now in Cognitive Behavior Therapy for Anxiety and special training to diagnose Autism.
Dr. Perez is a wife and mother of two busy boys. She is famous amongst her patients for her ability to remember the details of their child's medical histories and for building deep, meaningful relationships with patients and their families. She truly believes that in that trusting relationship, Pediatricians and Parents can best help kids thrive.

At MP Pediatrics, we are a small team caring for a small panel of patients to best serve you.
We offer three options for payment
Commercial PPO Insurance
We will continue to accept traditional Commercial PPO insurance, with whom we are in network, Blue Cross Blue Shield of Illinois, Aetna, United Health Care, UMR, Cigna, and Tricare.
Appointments: Patients using their PPOs will continue to receive the same excellent care they have been accustomed to at MP Peds. These visits will continue to be billed to insurance based on the level of care, as always, with co-pays, co-insurance, deductibles billed to the family. At this point in time, most PPOs are NOT covering video visits, so this will limit their availability to these families.
Communication: We ask that all families download set up Spruce, our HIPPA compliant communication tool. This will be the main way to communicate with the office, especially after hours. It is the fastest way to ask a medical question, get labs results, and request appointments.
Vaccines/Labs: will continued to be billed via insurance
As Needed Appointments, Cash Fee Schedule
This option will be removed once we have reached capacity for our patient panel
For patients who do not have or do not want to use commercial PPOs, we have a simplified fee cash fee schedule.

Vaccines/Labs: for families who have commercial insurance, we can bill these to your commercial insurance. For those families without commercial insurance; you can get vaccines at the local health department or choose to pay for them through us. Labs will have additional discounted fees or can be done elsewhere if you choose.
MP Premier
Direct to Patient/Membership option
For patients who do not have or do not want to use commercial PPOs, we now offer a membership. These patients will pay a monthly fee, per child, committed for a year. No co-pay, no co-insurance, no deductible, no varying fees based on level of care. The monthly fee includes;
- all well child visits
- all follow up visits (except for those done through our other business MPower; IHBLT, CBT, Autism Evaluations)
- all sick visits, with the option for video calls, phone calls or direct text messages and the opportunity for home visits when available (for an additional fee)
- direct text messaging with Dr. Perez for questions including after hours
Communication: We ask that all families download and set up Spruce, our HIPPA compliant communication app. Families in the membership will have additional access to direct text messaging to Dr. Perez including for after hour urgent questions.
Vaccines/Labs: for families who have commercial insurance, we can bill these to your commercial insurance.
